Panel 4: Yeahp, the station’s peculiar form of artificial gravity was first indicated by a logo on a bulkhead back at the beginning of the volume, and then again only a few pages ago. Both times, though, the secondary display under the main GRAVITY C logo read “ACTIVATED,” as opposed to “MALFUNCTIONING” as we see here.
Panel 5: To be honest, any foreign-object impact or cutting force severe enough to tear open the presumably durable materials of a Space Activity Suit would likely have done Very Bad Things to Mindf**k’s body—but, hey, just maybe something sharp could’ve struck her at an angle that tore open only the suit and not her flesh. Unlikely, perhaps, but not impossible.
